Money Follows the Person (MFP)
The Money Follows the Person Program is state and federally funded and enables Medicaid recipients, living in long term care facilities such as nursing homes to move back to their communities. Transition Coordinators provide assistance to connect clients with State of CT Department of Social Services waivers, resources and community supports. If individuals transitioning need housing, Housing Coordinators will assist by locating acceptable apartments or other living arrangements and applicable financial assistance programs. Some funds may be available to assist individuals to set up their household in the community or to make accessibility modifications. Assistive technology is available to help individuals be as independent as possible in the community.
