The CT Home Care Program for Elders

The Western Connecticut Area Agency on Aging, Inc. is pleased to announce that we have been designated as a provider for the CT Home Care Program for Elders as of July 1, 2013 for Waterbury residents.

The CT Home Care Program for Elders is a Program designed to help Elderly Persons remain safely in their home with the proper supports and services in place rather than Nursing Home placement. In order to qualify for the CT Home Care Program for Elders the person needing services must be at least 65 years old, be a CT resident, be at risk for nursing home placement, require assistance in at least 2 areas of their daily functioning, as wel as meet the program's financial eligibility requirements. You may qualify for services if your assets are below $35,766 as an individual and $47,688 as a married couple. There is no income limit.

Individuals may be required to contribute towards the cost of care based on their monthly income. You may be subject to a 9% cost share.

The CT Home Care Program for Disabled Adults  

The CT Home Care Program for Disabled Adults (CHCPD) Pilot Program offers a package of home-based services to a maximum of 100 persons, age 18 – 64, with a primary diagnosis of a degenerative, neurological condition who are not eligible for other programs, who need case management and other supportive services and are not eligible for other programs.  In addition to age and diagnosis requirements, applicants must be at risk for nursing home placement, require assistance in at least 3 areas of their daily functioning, as well as meet the program's financial eligibility requirements. Applicants will be screened for financial eligibility.  The asset limit for a single applicant is $35,766.00;  $47,688.00 for a married applicant.  There is no income limit. 

Individuals may be required to contribute towards the cost of care based on their monthly income.

 Exclusion Criteria

The following are not eligible to participate in this pilot program:

  • Persons who would qualify for the Personal Care Assistance Waiver
  • Persons whose primary disability is Mental Illness or Mental Retardation
  • Persons who are either Medicaid active or eligible for Medicaid
