Job Description:
The Service Navigator is as full-time position where you will support complex navigation and care management services with a focus on complex navigation to relieve the isolating and negative effects of the pandemic and connect clients to long-term services & supports. You will conduct detailed assessment and options counseling and provide timely information, referral, and application assistance over the phone, in client’s homes, on-site, or via email. This position will support the needs of municipal agents, senior center directors and community focal points through case consultation.
Qualifications:
BA/BS degree preferred with some background in senior services. Associate degree acceptable with 5-7 years of work or intensive volunteer experience in directly working with seniors. Bilingual, Spanish speaking preferred. Direct experience in human services preferred with an emphasis on the elderly. Knowledge of and experience with aging and long-term options services, programs and issues including detailed knowledge of caregiver issues and services. Must be CHOICES and AIRS trained OR able to participate in these courses and become certified in the first four to six months of employment in this position. Strong verbal and written communication skills needed with the ability to interview and counsel with empathy. Computer proficient in all Microsoft Office applications. Valid CT Driver’s license and reliable transportation. Ability to lift/carry 5-10 lbs., work in diverse, non-climate-controlled environments.
Salary: $24.73 - $27.47 per hour
