Congregate Housing Services (CHSP)
The CHSP program is offered in subsidized housing with care planning, coordination and monitoring of services for seniors and persons with disabilities financed by HUD, US Department of Agriculture, and the WCAAA through the State of Connecticut, Department of Social Services. Clients are asked to contribute a 25% of the cost of services, up to 20% of their adjusted gross income.
Depending on need, services may include:
- Congregate meals / Meals on Wheels
- Personal emergency response systems
- Adult day care
- Transportation
- Foot care
- Personal care assistance or homemaker

WCAAA's Professional Assessment Committee (PAC) meets regularly to approve services for clients after assessments are made. These cases and care plans are reviewed anonymously at the meetings by the PAC committee members and services may be approved or modified at that time.
