Money Follows the Person (MFP) Transition Coordinator

General Statement of Duties:
Western CT Area on Aging, Inc. is looking for a Full Time (35 hrs/wk) MFP Transition Coordinator who will generally be responsible for transitioning individuals from a skilled nursing facility back into the community safely. MFP services are to assist in the participants' successful transition and integration into non-institutional living arrangements in various communities. This position focuses on risk mitigation and prevention of re-institutionalization for seniors and younger disabled individuals through collaboration with a care manager implementing services and a housing coordinator ensuring adequate housing. Together with these positions, the transition coordinator is assisting with various applications for community resources, purchasing of food and other items to furnish the new residence. Other tasks include obtaining transportation to and from appointments, working with services providers, getting new identification cards. A transition coordinator will provide ongoing monitoring, advocacy and support with consideration to special person centered preferences when possible. Participation in various interdisciplinary meetings concerning the individual’s wellbeing and any status changes is necessary and may require travel beyond the WCAAA region. Remote work is available based upon completion of training curriculum and demonstrated skill proficiency after 6 months of employment.
Qualifications:
Associate’s degree minimum educational level. Bachelor’s Social Work or a related field preferred. One year of demonstrated related experience working in health and human services as well as familiarity with local resources. Vehicle and valid CT driver’s license required as this position requires travel. Organizational skills, excellent time management, professional interpersonal skills, and knowledge of community resources. Engaging individuals in their homes, a nursing or other skilled facilities will be required as needed. Physical ability to lift and carry 5‐10 Ibs. such as a laptop computer and navigate stairs in the absence of elevators may be required if accommodations are not available in certain housing settings. Second language is a plus.
