Care Manager Supervisor of the Strategic Unit

Supervisor: Director of Home Care
General Statement of Duties:
Full time position aimed at supervising, evaluating and tracking contract deliverables executed by assigned staff with Money Follows the Person, Community First Choice and My Care Options programs. This position focuses on risk mitigation and prevention of reinstitutionalization for seniors and younger disabled individuals through care management services who reside inside a skilled setting or in the community. Daily tasks include conducting team meetings, acting as a liaison with DSS to discuss and process cases, audit case work before submission to DSS, participate in CO Strategy meetings concerning the progress and/or development of assigned programs. This position is also tasked with supporting team members in any capacity necessary to complete MFP transitions, ensure deadlines are met and paperwork is complete. Advisement on Care Plans, goal development and implementation of services will support team members and an individual to remain safely in their homes. Provide ongoing monitoring, advocacy and support within an atmosphere of person-centered planning. Participation in various interdisciplinary meetings concerning the individual’s wellbeing and any status changes is necessary and may require travel beyond the WCAAA region. Remote work is available based upon completion of training curriculum and demonstrated skill proficiency after 6 months of employment.
Qualifications:
Bachelor’s or master’s degree in Social Work or a related field. LMSW or LCSW preferred. Bachelor’s degree in Nursing with current active CT license; two years of demonstrated related experience working in health and human services with focus around assessment and care plan development as well as supervisory experience or demonstration of supervisory qualities. Vehicle and valid CT driver’s license required as this position requires travel. Organizational skills, excellent time management, professional interpersonal skills, and knowledge of community resources. Excellent flexibility and adaptability skills required to manage staff and procedural changes. Candidate must be in good standing Office and field work in client homes, a nursing or other skilled facilities will be required as needed. Physical ability to lift and carry 5-10 Ibs. such as a laptop computer and navigate stairs in the absence of elevators may be required if accommodations are not available in certain housing settings. Second language is a plus.

Salary Range: $58,000-$60,000
