Our WCAAA Service Navigators offer comprehensive support to older adults (60+) and their caregivers, as well as individuals with disabilities (18-59) and their caregivers. The program provides information, education, and guidance to help navigate complex situations. Services include benefits counseling, application assistance, case consultation, and person-centered planning, which encompasses options counseling, case management, and short-term counseling. We collaborate with other community partners to enhance access to a range of services funded by state, federal, and local programs, all aimed at improving quality of life. These services include housing, legal and financial assistance, in-home services, employment, home modifications, long-term support, and protective services. Support is available in person, by phone, at 203-757-5449 ext. 120, or via email at [email protected].
Our team would also love to come out and give an "All About Us" presentation at your location, if you are a senior center located in one of our 41 towns. It's a great opportunity to learn more about our programs and services, and how we can support your community. If you're interested in scheduling a presentation, feel free to reach out to us via email or phone. We’d be happy to coordinate a time that works for you.
