(WCAAA) is seeking a Part-Time (15 hours/week) Project Coordinator to support the implementation of the Danbury Transportation Needs Assessment, a 12-month initiative focused on identifying transportation barriers impacting older adults, individuals with disabilities, caregivers, and other community members in Danbury.
The Project Coordinator will assist with project planning, community outreach, survey distribution, listening sessions, data collection, stakeholder communication, and reporting activities. This position will collaborate closely with WCAAA leadership, community partners, transportation providers, healthcare organizations, senior centers, housing sites, and local stakeholders.
The goal of this project is to develop a professional needs assessment report with practical recommendations to improve mobility, independence, healthcare access, and quality of life for Danbury residents.

Project Coordination & Administration
- Support implementation of the 12-month project work plan
- Coordinate meetings with staff, project partners, and stakeholders
- Maintain project records, outreach logs, schedules, and documentation
- Assist with tracking timelines, deliverables, and grant-related activities
- Help ensure alignment with approved project goals and budget
Community Outreach & Engagement
- Coordinate outreach to older adults, individuals with disabilities, caregivers, and community residents
- Work with senior centers, housing sites, healthcare providers, nonprofits, and community organizations to promote participation
- Support multilingual and accessible outreach efforts
- Assist with planning and facilitating approximately 4–6 listening sessions or focus groups
- Coordinate participant supports, accommodations, and follow-up communication
Survey & Data Collection
- Assist with development and distribution of community surveys
- Help achieve approximately 150–200 survey responses from diverse Danbury residents
- Coordinate collection of transportation-related information from partner agencies
- Document themes and feedback from listening sessions and stakeholder conversations
Reporting & Communication
- Organize survey results, demographic information, and community feedback
- Support development of transportation recommendations and findings
- Assist with drafting and formatting the final Transportation Needs Assessment Report and Executive Summary
- Help coordinate presentations and dissemination of project findings
- Prepare project summaries and materials to support future planning and grant opportunities
Requirements: Required Qualifications
- Associate’s degree required in human services, public health, social services, community development, nonprofit management, public administration, transportation planning, urban studies, or a related field
- Minimum of 2 years of experience in project coordination, community outreach, public health, aging services, disability services, nonprofit programs, transportation-related services, or a related field
- Strong organizational and communication skills
- Ability to manage multiple priorities and collaborate with diverse partners
- Ability to work respectfully and effectively with older adults, individuals with disabilities, caregivers, and multicultural communities
- Proficiency with Microsoft Office, spreadsheets, online survey tools, and virtual meeting platforms
- Driver’s license and ability to travel locally within the Danbury/Western Connecticut area
Preferred Qualifications
- Experience with surveys, focus groups, listening sessions, or community needs assessments
- Familiarity with transportation access issues, ADA accessibility, paratransit, or aging services
- Experience working with municipal agencies, transit providers, or nonprofit organizations
- Bilingual or multilingual skills strongly encouraged
- Familiarity with data analysis, GIS mapping, or data visualization tools is a plus
Compensation
- $30.00 per hour
- Approximately 15 hours per week for 52 weeks

The Benefits Specialist plays an essential role in helping community members understand their options, complete screenings, navigate benefits programs, and access needed services. This 35 hour a week position combines direct client support, outreach, data management, and community education.
Key Responsibilities
- Complete CHOICES New Team Member Training and participate in all required training, webinars, and meetings
- Provide individualized Medicare and Medicaid counseling in office and community-based settings
- Conduct client screenings using approved tools and provide follow-up guidance
- Deliver educational presentations to seniors, caregivers, and professional partners
- Accurately enter and maintain client information in case management systems
- Develop and maintain educational materials and outreach toolkits
- Support and coordinate volunteer activities in office and field settings
- Assist with preparation of monthly, quarterly, and performance benchmark reports
- Maintain strict confidentiality and compliance with federal, state, and organizational requirements

General Statement of Duties:
The Care Manager Assistant is a full-time 35hr/wk in-office position that is supportive of the entire CT Home Care Program (CHCP) unit and based on related responsibilities. Care Manger Assistant’s (CMA’s) may need to interact directly with clients upon direction in-office and in the field to:
- Creates, updates and produces clients’ charts.

Full time position (35 hrs/wk) aimed at risk mitigation and prevention of institutionalization for seniors and younger disabled individuals through care management services. Daily tasks include conducting comprehensive in-home assessments of seniors or younger disabled individuals to identify strengths and areas of need. Care Plan and goal development and implementation of services will help support an individual to remain safely in their homes. A care manager will provide ongoing monitoring, advocacy and support within an atmosphere of person centered planning. This position acts as the primary service contact with the senior or younger person with a disability (or families). Care Managers may be required to help the client/family with required paperwork to secure and maintain benefits including but not limited to Medicaid, Medicare, housing, and transportation. Participation in various interdisciplinary meetings concerning the individual’s wellbeing and any status changes is necessary and may require travel beyond the WCAAA region. Remote work is available based upon completion of training curriculum and demonstrated skill proficiency after 6 months of employment.
Requirements: Qualifications:
Candidates must be willing to work on site from the office during the initial 6-month period in between field work. Remote work is available to eligible employees after successful completion of this period. Applicants must possess Bachelor’s or Master’s degree in Social Work or a related field; LMSW or LCSW preferred, or a Bachelor’s degree in Nursing with current active CT license. All applicants must have two years of demonstrated related experience working in health and human services. Acquired Brain Injury Care Managers must possess an LMSW, LCSW, or a Master’s degree in Human Services, Counseling or Rehabilitation Counseling, in addition to two years demonstrated experience. Familiarity with computers is required as well as the capacity to carry a laptop. Vehicle and valid CT driver’s license required as this position requires travel. Organizational skills, excellent time management, professional interpersonal skills, and knowledge of community resources. How much does the CHOICES Training Cost?
CHOICES New Volunteer training is free for volunteers and $125 for in-kind (paid) professionals. The training consists of one half-day orientation and four full-day training sessions. *Insurance agents, brokers and financial planners are not eligible to participate in the training. *

Apply NowDescription: Seeking volunteers for the Professional Audit Committee (PAC) at Western CT Area Agency on Aging (WCAAA) to clinically evaluate client records for active consumers under the Connecticut Home Care Program. The purpose of these audits is to glean a non-biased review of documentation, ensuring risk assessment and mitigation, and that client can remain living safely in the community. Volunteers will work in conjunction with the QA Coordinator to meet an annual requirement of chart reviews. Interested volunteers will be able to commit up to 2 hours per month in office reviewing charts. This time is flexible and set by the volunteer.
Requirements: - Volunteers with a Bachelor’s degree in Nursing or a Bachelor’s in Social Work or related field
- Volunteers must have experience in health care or human services
