CHSP

The CHSP program is available in designated subsidized housing sites to individuals who are 62 years of age or older or adults with disabilities (regardless of age) who require assistance with at least three activities of daily living.  These activities may include:

  • Eating (cooking and/or preparing food)
  • Bathing (getting in and out of the tub/shower)
  • Grooming (hair and nail care, personal hygiene)
  • Dressing (needing occasional assistance)
  • Transferring (getting in/out of bed or chairs, walking, toileting)
  • Household management (housework, grocery shopping, bill-paying, laundry)

Following a home assessment and discussions, the RSC and resident develop an individualized care plan which may include:

  • Care management
  • Congregate meals
  • Personal care assistance
  • Homemaker services
  • Companion services
  • Foot care
  • Bill-paying assistance
  • Personal emergency response service

The plan is then presented anonymously to the Professional Assessment Committee (PAC) for approval, denial or modification.  The cost of services are shared by the participant, a Housing and Urban Development (HUD) grant, WCAAA and the State Department on Aging.

  • Torringford West (Torrington)
  • Chestnut Grove (Winsted)
  • Greenwoods Garden (Winsted)
  • George B. Lewis I & II (Naugatuck)
  • Grace Meadows (Southbury)
  • Nunnawauk Meadows (Newtown)
