    • Oversees the design, coordination, and implementation of WCAAA’s wellness and healthy aging initiatives.
    • Manages health counseling, case management, nutrition education, and evidence-based Chronic Disease Self-Management Education (CDSME) workshops.
    • Ensures programs meet federal Title III requirements, grant obligations, and agency standards while monitoring outcomes and preparing reports for WCAAA leadership and funding sources.
    • The Coordinator receives referrals from healthcare providers, conducts individualized assessments, and develops wellness support plans that reflect physician recommendations and client goals.
    • With direct engagement with older adults, will connect them to vital community resources—including transportation, nutrition, exercise, CDSME workshops, and home support while addressing barriers to participation.
    • Track client progress and outcomes over time.
    • Under the oversight of WCAAA’s Registered Dietitian, the Coordinator delivers education sessions, documents services in agency systems, and collaborates with nutrition providers, the Board of Directors, and stakeholders to ensure alignment with the approved Nutrition Education Plan.
    • Acts as a liaison between WCAAA, Sharon Hospital, community health centers, and local providers.
    • Builds referral networks that strengthen clinical-community integration.
    • Conduct outreach to expand awareness of programs, generate referrals, and ensure services are accessible to diverse populations across the region.
    • Promote public awareness of WCAAA services by publicizing workshops and programs through community presentations, press releases, flyers, and online platforms such as the CT Healthy Living website.
    • Responsible for maintaining accurate and timely client and program records in accordance with HIPAA and agency policies. This includes producing reports that demonstrate accountability, outcomes, and effectiveness.

    Compensation

    This is a full-time 35 hour a week, grant-funded position with a competitive salary and benefits as determined by WCAAA policy. Continuation beyond the initial grant year is dependent on performance outcomes and funding availability.
