    General Statement of Duties:

    Full time position (35 hrs/wk) aimed at risk mitigation and prevention of institutionalization for seniors and younger disabled individuals through care management services. Daily tasks include conducting comprehensive in-home assessments of seniors or younger disabled individuals to identify strengths and areas of need. Care Plan and goal development and implementation of services will help support an individual to remain safe in their homes. A care manager will provide ongoing monitoring, advocacy and support within an atmosphere of person-centered planning. This position acts as the primary service contact with the senior or younger person with a disability (or families). Care Managers may be required to help the client/family with the required paperwork to secure and maintain benefits including, but not limited to, Medicaid, Medicare, housing, and transportation. Participation in various interdisciplinary meetings concerning the individual’s wellbeing and any status changes is necessary and may require travel beyond the WCAAA region. Remote work is available based upon completion of training curriculum and demonstrated skill proficiency after 6 months of employment.

    Requirements:
    • Candidates must be willing to work on site from the office during the initial 6-month period in between field work. Remote work is available to eligible employees after successful completion of this period.
    • Applicants must possess Bachelor’s or Master’s degree in Social Work or a related field; LMSW or LCSW preferred, or a Bachelor’s degree in Nursing with current active CT license.
    • All applicants must have two years of demonstrated related experience working in health and human services.
      • Acquired Brain Injury Care Managers must possess an LMSW, LCSW, or a Master’s degree in Human Services, Counseling or Rehabilitation Counseling, in addition to two years demonstrated experience.
    • Vehicle and valid CT driver’s license required as this position requires travel.
    • Organizational skills, excellent time management, professional interpersonal skills, and knowledge of community resources.

    Western CT Area on Aging, Inc. is looking for a Full Time (35 hrs/wk)  MFP Transition Coordinator who will generally be responsible for transitioning individuals from a skilled nursing facility back into the community safely. MFP services are to assist in the participants' successful transition and integration into non-institutional living arrangements in various communities.  This position focuses on risk mitigation and prevention of re-institutionalization for seniors and younger disabled individuals through collaboration with a care manager implementing services and a housing coordinator ensuring adequate housing. Together with these positions, the transition coordinator is assisting with various applications for community resources, purchasing of food and other items to furnish the new residence.  Other tasks include obtaining transportation to and from appointments, working with services providers, getting new identification cards. A transition coordinator will provide ongoing monitoring, advocacy and support with consideration to special person centered preferences when possible. Participation in various interdisciplinary meetings concerning the individual’s wellbeing and any status changes is necessary and may require travel beyond the WCAAA region. Remote work is available based upon completion of training curriculum and demonstrated skill proficiency after 6 months of employment.

    Requirements:
    • Associate’s degree minimum educational level.
    • Bachelor’s Social Work or a related field preferred.
    • One year of demonstrated related experience working in health and human services as well as familiarity with local resources.
    • Vehicle and valid CT driver’s license required as this position requires travel.
    • Organizational skills, excellent time management, professional interpersonal skills, and knowledge of community resources.
    • Engaging individuals in their homes, a nursing or other skilled facilities will be required as needed.

    Individuals who would like to make a positive difference in the lives of Medicare beneficiaries. Interested applicants should have a variety of skills including listening, computer, internet skills, etc. CHOICES volunteer roles include Administrative Support, Counselor, Open Enrollment Counselor, Outreach Volunteer, and Presenter. Certification requirements include: agreeing to adhere to CHOICES Volunteer Risk & Program Management (VRPM) policies and procedures; passing CHOICES exams; signing the CHOICES Memorandum of Understanding; attending CHOICES Update trainings; and reporting SHIP related performance activities.

    How much does the CHOICES Training Cost?

    CHOICES New Volunteer training is free for volunteers and $125 for in-kind (paid) professionals. The training consists of one half-day orientation and four full-day training sessions. *Insurance agents, brokers and financial planners are not eligible to participate in the training. *

    Seeking volunteers for the Professional Audit Committee (PAC) at Western CT Area Agency on Aging (WCAAA) to clinically evaluate client records for active consumers under the Connecticut Home Care Program. The purpose of these audits is to glean a non-biased review of documentation, ensuring risk assessment and mitigation, and that client can remain living safely in the community. Volunteers will work in conjunction with the QA Coordinator to meet an annual requirement of chart reviews. Interested volunteers will be able to commit up to 2 hours per month in office reviewing charts. This time is flexible and set by the volunteer.

    Requirements:
    • Volunteers with a Bachelor’s degree in Nursing or a Bachelor’s in Social Work or related field
    • Volunteers must have experience in health care or human services
    • Volunteers will adhere to confidentiality requirements and be conflict free from any business partners working with the Connecticut Home Care Program and/or WCAAA
