Position Available - Transition Coordinator - Money Follows the Person

This full time position is responsible for a variety of activities focused on transitioning eligible nursing home residents to community living. Duties include but are not limited to meetings with residents, providing assistance to residents in benefits’ applications, facilitation of transition plan including services. Transition Coordinators must participate in CADER training and successfully complete that certification process.

Requirements:

Bachelor’s degree with minimum of five years’ experience in non profit agency; familiarity of 41 towns of western CT area and various computer applications necessary. Car and valid driver’s license required as this position involves extensive travel.  Transition Coordinator must be able to lift and carry 5 – 10 lbs such as lap top computer, other equipment.
