Position Available - Specialized Care Manager - Money Follows the Person Program

Specialized Care Manager - Money Follows the Person Program: this position supervises the WCAAA's MFP team of Housing & Transition Coordinators with primary responsibility for in person assessments of nursing home residents deemed eligible for MFP benefits.  This person would be responsible for conducting a Universal Assessment, care planning, assessing risk and predicting and resolving barriers to the resident's return to the community within DSS budgetary/programmatic guidelines. This position would also entail short-term case management before transitioning to their slotted program. BA/BS required with some demonstrated knowledge of assessment protocols and benefits available in CT. Car and valid driver's license necessary as this position involves extensive in-state travel. Ability to carry at least ten pounds also required as this position utilizes a computer.
